What is considered PHI? Protected Health Information (PHI) is all information that can be used to identify an individual. This includes but is not limited to Name, Date of Birth, Address, Phone Number(s), fax number, e-mail, Health Plan ID numbers, Medicare beneficiary ID, prescription numbers, medical records, pharmacy records, insurance records, etc.
How do we obtain your PHI? This information is provided to us securely by our clients with the sole intent of providing care and service to the extent of our contractual obligations. Records may be requested from your provider, Pharmacy or Health Plan to fulfill our duties. This information is never purchased or sold and may only be used to provide service and care to individuals specified by our clients.
How do we use your PHI? Your personal information is used to fulfill contracted services by the client such as but not limited to: medication adherence, comprehensive medication reviews, medication related problems, gaps in care, screening scheduling, etc.
How do we share your PHI? We do not share your PHI outside of your designated care team such as your Health Plan, Provider(s) or pharmacy. All information is transmitted safely and securely in accordance to HIPAA guidelines.
You have rights when it comes to your PHI. If you have any questions or concerns about your rights, please reach out to us immediately. Right to request restrictions on certain uses and disclosures of your PHI. Right to request a copy of your PHI. Right to amend or change your PHI. Right to receive a history of disclosures of your PHI. Right to request alternate means or locations your PHI is disclosed to you.
Electronic Communications. We do not contact you about your activity on our website. We may reach out to you on behalf of your health plan as part of services your health plan provides for you. By giving your health plan or HHH your contact information, you have agreed that your health plan and its contractors such as HHH may contact you for benefit and care coordination purposes. HHH communications will not be done by automated dialing equipment and/or artificial or prerecorded messages. We do not charge for our short message service (SMS); however, regular message and data rates may apply from your mobile carrier. We are not responsible for any data transmission fees. You can opt out at any time by replying “STOP” to a text message, call us or submit a Privacy Complaint Form. This opt-out process does not apply to live phone calls or emails, which may continue in case we need to reach you.
Please note that the information you provide to us through the website forms may not be secure (not encrypted). Email via the Internet and through text messaging, such as a device’s SMS, or other electronic means may not be secure (not encrypted). There are risks associated with unsecure communications that you should consider. These risks include (without limitation) that an unauthorized person may get access to these communications. By engaging us in unsecure communications, you agree to accept these risks and consent to receive unsecure communications in response from us. If you engage us in unsecure electronic communications, please do not include any sensitive Information in your communication. SMS consent or phone numbers for the purpose of SMS are not shared.
Privacy Policy Updates. HHH holds the right to update or change their Privacy Policies to fit the needs of our clients, their members, and ongoing changes to rules and regulations. We reserve the right to make any changes to these policies, at any time, without prior notice to you. Changes made to Privacy Policies will be published on the website and effective immediately unless otherwise specified.
